ENROLLMENT PROCEDURES
CHECK OFF LIST
A child may be enrolled by a parent, foster parent, caseworker, caregiver with whom the child resides, or any other person with a "charge or control" of the child.
- Proof of Residency - Copy of a deed, lease, utility bill,
vehicle registration, driver's license or Dept. of
Transportation ID Card.
- Court Order/Custody Agreement - If there is a court
order/customer agreement involving the student - we are
required to have papers on file.
- Proof of Child's Age - we are required to request proof of
age for all students entering the district. Acceptable
documents include birth certificate, passport, or
baptismal certificate.
- Guardianship Papers - If student is living with a legal
guardian other than legal parent - we are required to
have legal, notarized guardianship papers on file.
- Parent must sign a sworn statement about serious
discipline problems at prior schools.
- Immunization Records - Current and up to date. Student
cannot start without these.
- Please complete emergency cards on both sides with parent/guardian signature.
- Please complete and sign all attached forms.
