The
following state requirements must be met prior to the first day of school for
the following students:
Incoming
third-graders: Dental exam dated within one year prior to the student’s first day of third grade.
Incoming
sixth-graders: Physical exam dated within one year prior to the student’s first day of sixth grade. Dr. Matthew Vasil, our school physician, can also complete this exam during the school year if preferred.
Incoming
seventh-graders: Vaccinations given after the student’s 11th birthday:
A letter, statement or
record from your health provider showing each dose of vaccine and the full date
(month/day/year) administered must be submitted.
Incoming
seventh-graders: Dental exam dated within one year prior to the student’s first day of seventh grade.
Incoming
juniors: Physical exam dated within one year prior to the student’s first day of eleventh grade. Dr. Matthew Vasil, our school physician, can also complete this exam during the school year if preferred.
Incoming seniors: Second dose of the MCV (meningitis vaccine) after
student's 16th birthday
A letter, statement or record
from your health provider showing each dose of vaccine and the full date
(month/day/year) administered must be submitted.
Completed
forms and vaccination documentation can be dropped off at any building or
emailed to the appropriate school nurse as follows:

Medical
and religious exemptions are accepted.
